Hand Case 17 ED Management
Dorsal plate fractures involving < 30% of the joint surface can be managed with splinting in extension for 3-4 weeks, followed by buddy taping for 2 weeks. Fractures that involve > 30% of the articular surface should be treated with non-emergent ORIF upon follow up with a hand surgeon. Improper splinting or non-operative management can lead to shortening and contracture of the central tendon with hyperextension at the PIP, causing boutonniere deformity.
