History may be significant for contact sports, motor vehicle accidents, or sudden muscle contractions (as in straining to lift heavy objects). Most patients will present with acute posterior neck pain that is tender to palpation over the affected spinous process. Pain may be worse with active ranging of the neck with rotation, flexion, and extension. Pain should not be radicular and neurologic exam should be intact.
